We’re a home from hospital project, providing welfare calls and support for up to 6 weeks post discharge.
We help local people to find volunteering roles & also support organisations in finding volunteers.
We engage with patients to support them. We can signpost or refer to support in the community.
Project to help VCFSE organisations build digital inclusion & resilience for them and their communities.
